CHEMICAL RELEASE FORM
I give permission for my stylist to perform the hair chemical service discussed/booked and will hold
them unaccountable from any liability that may result from this service. I understand my stylist will
take every precaution to minimize negative reactions. If in the case something was to occur, I will
consult my stylist immediately. I understand the refund policies stated below & understand I paid for
time and service done by provider. I understand hair color can, and in some individuals, cause an
allergic reaction. I've made my stylist aware of any concerns (type of water, medications, previous hair
color, health concerns, etc.) I understand I can ask my stylist to do a patch test prior to receiving the
service. If customer is under the age of 18, a parent or guardian must authorize services.
SALON POLICIES
Cancellation/ No show policy:
We take pride in reserving your requested time slot for your beauty needs! In order to book an
appointment with us we request a card on file. We understand there are times you may need to
reschedule! We would really appreciate 48 hour notice to be able to let clients on the wait list know.
Failure to cancel within 24 hours will result in a 50% charge & a no show or same day cancel will result
in 100% charge of the selected service. Thank you for respecting our time & business!
Hair Services:
Dissatisfaction in service must be expressed within five days. Services are corrected by the initial
provider to fix, correct, & work with a client to achieve a satisfactory resolution. Requesting another
service provider for a fix is contingent upon the situation. Monetary refunds are not rendered on
services. We take pride in our work and want all our clients satisfied.
Retail Products:
Products that are "gently used", damaged, or defective may be exchanged for another product &
exchanged within 15 days. This exchange is pending management approval. This does not apply to
shampoos & conditioners, lotions, or brushes.
Gift Certificate:
All gift certificates are final sale. Cash or checks are suggested for purchasing gift certificates due to the
fact some stylists are independent contractors and have their own processing system for transactions.
(Unless told otherwise by their service provider or service provider is present.) Balances on gift
certificates can be used towards future appointments and tips can be used on gift certificates. Gift
certificates cannot be used towards retail.
Bridal policy:
There is a deposit of $150 to "save the date with Color Bar" which will be applied towards bride's
services day of. The deposit is a nonrefundable deposit once date is booked. 6 wks prior to wedding we
require 50% of bridal party services. Charges will be applied if there's a change within 2 weeks or less of
wedding date. 50% charge 2 weeks or less & 100% charge for same day no show.
